The Urbane Scrubs “WORLD OF COLORS” Sweepstakes begins at 12:01 a.m., Central Time (“CT”) on JANUARY 5, 2015 and ends at 11:59p.m., (“CT”) on JANUARY 29, 2015 (“Sweepstakes Period”). Sweepstakes is sponsored by Urbane Scrubs, P.O. Box 516, Olive Branch, MS 38654 (the “Sponsor”), who is solely responsible for all aspects of this Sweepstakes.
ELIGIBILITY: Sweepstakes is open to all legal residents of the United States and District of Columbia who are 18 years of age or older as of the Sweepstakes Start Date.
HOW TO ENTER: Eligible participants can be entered into the Sweepstakes by going to the Sponsors’ Facebook page at www.facebook.com/UrbaneScrubs. Participants do not have to be a current Facebook Fan of Urbane Scrubs. Each entrant will be REQUIRED to provide: FIRST NAME, LAST NAME, E-MAIL ADDRESS, OCCUPATION, SHIPPING ADDRESS, CITY, STATE, ZIP CODE and MOBILE NUMBER. As an OPTION, entrant can “Tell us why U love Urbane Scrubs”. Any submitted quote, name or likeness may be featured via any or all Urbane Scrubs marketing or social media entities including but not limited to: Facebook, Twitter, Pinterest, Email, Direct Mail, catalog, etc. No further compensation will be remunerated for use of “quote, name or likeness” in any or all forms of media, in perpetuity. Submission of entry form constitutes personal release. Grand Prize and Bonus Prize will both be announced on Friday, January 30, 2015, on the Sponsor’s Facebook Page. All eligible entrants will be considered to win.
All REQUIRED FIELDS on the online registration form must be completed in order for an entry to be eligible. LIMIT one entry per person, per email address, per household, per day. All eligible entrants, determined by cumulative entry during the entire sweeps period, are eligible for the Bonus Prize drawing.
